Ihren Durchbruch feierte Milano 1983 mit der Rolle der Samantha in der Fernsehserie "Wer ist hier der Boss?". Die Serie lief von 1984 bis 1992 und war sehr erfolgreich. Später spielte sie von 1997 bis 1998 Jennifer Mancini in "Melrose Place". Von 1998 bis 2006 verkörperte sie Phoebe Halliwell in der Serie "Charmed – Zauberhafte Hexen". Neben ihrer Schauspielkarriere veröffentlichte Milano zwischen 1989 und 1992 fünf Musikalben, die in Japan und Südkorea mit Platin ausgezeichnet wurden. Im Jahr 2007 präsentierte sie ihre Modekollektion "TOUCH by Alyssa Milano", die sich an weibliche Baseballfans richtete. Milano engagiert sich auch sozial. Seit 2003 ist sie "Celebrity Ambassador" für UNICEF und seit 2007 für das "Global Network for Neglected Tropical Diseases". Sie setzt sich für Tierrechte ein und unterstützt Organisationen wie PETA und Sea Shepherd. Im Oktober 2017 initiierte sie nach dem Weinstein-Skandal auf Twitter den Hashtag #MeToo, um auf sexuelle Belästigung aufmerksam zu machen.
